• Sports Physicals and Athletic Clearance

     

    The first step in playing rugby is getting a sports physical, and getting registered and cleared through Athletic Clearance. There will be additional forms and paperwork to complete once the season begins. This is just the first step.

     

    All those wishing to play rugby must complete a school sports physical. To get a copy of the sports physical form, please click here. Additionally, those wishing to play must register and be cleared through Athletic Clearance. In order to make the registration process smoother, please have the player's student ID number and proof of insurance ready. Please click here to get started Athletic Clearance. You will need to select "rugby" as your sport. Once completed, the sports physical form and Athletic Clearance Confirmation Signature Page (signed by both the athlete and parent) must be turned into the Athletic Director's Office or the Athletic Director's mailbox BEFORE clearance can be finalized. You must be cleared through Athletic Clearance before being allowed to play or practice once the season begins.     If you already play a sport at Freedom High School and have been cleared through Athletic Clearance, please log into Athletic Clearance with your current user name and password. Select the current school year, Freedom HS as the school, and rugby as the sport. The first page will be the student information page. There will be a drop down menu; select the athlete's name associated with the log in. Most of the student information will automatically populate based on what was entered previously with your current sport. Please make sure all information is current before clicking "Save." Next, it will take you to "Physical Uploads." Click "Choose From Documents Library" to find your current school year's sports physical to select. You will be required to reenter all of the medical history questions as well as electronically sign all the forms (both athletes and parents). Please print off the Athletic Clearance Confirmation Signature Page, make sure it has all required signatures, and turn it into the Athletic Director's Office or the Athletic Director's mailbox BEFORE clearance can be finalized. NOTE - As of 10/1/20, the parent signature page must be uploaded to your Athletic Clearance profile. No paperwork is being handled at this time. You will also need to upload the COVID-19 waiver to your player profile, as well. 

     

    If you do not currently play a sport, you will need to create an account with Athletic Clearance prior to entering any information. You will be required to upload a copy of your sports physical and proof of medical insurance. Once you have completed all steps, and both you and your parents have electronically signed all forms, please remember to print off the Athletic Clearance Confirmation Signature Page. Once it has all required signatures, please turn it and your sports physical into the Athletic Director's Office or the Athletic Director's mailbox BEFORE clearance can be finalized. NOTE - As of 10/1/20, the parent signature page must be uploaded to your Athletic Clearance profile. No paperwork is being handled at this time. You will also need to upload the COVID-19 waiver to your player profile, as well. 

     

    It is a good idea to keep a copy of your sports physical as well as the signed Athletic Clearance Confirmation Signature Page just in case there is an error in receiving the files. If you are having trouble loading any of the required documents as PDFs, try loading them as a picture file instead. Also, Google Chrome is the best browser to use for Athletic Clearance.
