    Completed forms must be turned in to the Career Center to be processed. Please email me your completed work permit to gonzaleza@luhsd.net Or drop off work permit to A104.

    Work permits will be approved based on last semester's grades and attendance. Work Permits expire ~3 days after the new school year begins.

    PLEASE NOTE: COMPLETION OF A WORK PERMIT CAN REQUIRE 48 HOURS.


    TO COMPLETE THE REQUEST FORM PLEASE DO THE FOLLOWING:

    1  STUDENT COMPLETES SECTION REGARDING THE MINOR (Section 1).


    2 EMPLOYER COMPLETES THE SECTION REGARDING THE EMPLOYER (Section 2). PLEASE NOTE THAT ALL WORK PERMITS MUST HAVE A WORKERS COMPENSATION INSURANCE COMPANY LISTED.


    3  A PARENT OR GUARDIAN MUST SIGN/DATE THE FORM (Section 3).


    4 STUDENTS ARE REQUIRED TO HAVE AT LEAST A 2.0 GRADE POINT AVERAGE.


    5  RETURN WORK PERMIT REQUEST FORM TO THE CAREER CENTER FOR GRADE AND ATTENDANCE CHECK.

     


    The Career Center Coordinator verifies school enrollment, checks grades, fees due and attendance per Liberty Union High School District Administrative Regulation 5113.1(a). If the work permit is complete and a student is in compliance with AR5113.1(a), the work permit is issued.

    Administrative Regulation 5113.1(a) is excerpted below regarding approval/revocation of work permits. The entire regulation is available in the Career Center or Principal's office.


    1. No work permit shall be issued until the student's parent/guardian, foster parent or residential shelter provider has filed a written request with the district. (Education Code 49110)


    2. To help in determining the extent to which outside employment may be approved, the Superintendent's designee shall:

    a. Inspect the student's records for evidence of satisfactory grades and attendance.
    b. Confer with at least one of the student's teachers to determine whether the student appears to have the time, stamina, motivation and maturity to maintain academic progress while working.


    3. After issuing a work permit, the Superintendent's designee shall periodically inspect the student's scholastic and
    attendance records.


    4. The Superintendent's designee shall revoke a student's work permit whenever he/she determines that employment is
