Instructions: Please complete all sections of the form. When you have completed the form:

• Request an unofficial copy of your transcript from Career Center Coordinator.
• Make several copies of the request form and unofficial transcript.
• Give one copy of each to every person you request a letter of recommendation from.
• Please give the person writing your letter at least TWO WEEKS to complete your letter of recommendation, not including school breaks and weekends
• Please be sure to VERIFY if a letter of recommendation is required for the school you are applying to. Many schools do not require a letter.
