- Copy of current Parent/Guardian Government Issued ID
- Copy of Birth Certificate
- Proof of Home Address (Residence) Declaration of Residence
- Proof of Immunization Immunization Information
- Copy of most recent IEP or 504 (if applicable)
- Check out paperwork from previous school (if applicable)
Grade 9 Requirement: Proof of TDAP Immunization
